Defect Rate

Defect Rate measures the number of software defects discovered over time, relative to a baseline such as completed work, code changes, or production deployments. It reflects how often issues escape the development process and reach customers or QA environments.

Calculation

A defect is typically defined as any verified bug or issue that results in incorrect behavior, user impact, or failure to meet acceptance criteria. Defects may be found during internal testing, user acceptance, or after deployment. Common baselines include story points, pull requests, deployments, or development days.
